2 plastic tyre levers
2 threaded 16 g CO2 cylinders + 1 CO2 thread crack-pipe dispenser

If i'm going on longer rides (150km+), i'll chuck 1 more tube in my back jersey pocket.
Since i change/rotate chains every 1000km on my bike, and while they are off the bike I inspect them for damage, wear (stretch) and relube them; I don't bother carrying a breaker, links or a link holder.
I get my brake/gear cables replaced 6 monthly (excessive probably, but it is cheap insurance against failure), I don't need to carry spare cables.
I change my tyres approx every 6,000km so punctures are rare. If I get a run of punctures, it is usually something wrong with the tyre (as what happened last week). As a result I don't carry a pump.
