In a nutshell - if there are very big (80+) numbers in A grade, they split the grade into A+ and A grades ... partly because otherwise the large A grade would take ages to pass the other grades - which can cause accidents.
Mal says that lots of A graders like the challenge of riding with the pros - so rather than expecting to place, they try to finish with the pack, or achieve a breakaway or whatever.
Similarly, for the pros to get a good workout at a crit, they need a big field with lots of breakaways to chase, etc!
Bear in mind that the pros don't always win, too.
