Yep, goes between the cleat and shoes. It's a shim/spacer, so you might not need it, though I used one with my Specialized Sport MTB shoes for the first couple of sets of cleats. After about 10-15K kms on the shoes (can't remember exactly when, and it was mixed commute/road/mtb mileage), the rubber tread had worn down enough to not need it any more. I've always installed it with the bumps against the shoe surface.
